Digital Health Patient Navigator – Healthcare IT

Kaiser Permanente is seeking a Clinical Navigator to bridge clinical care and digital tools within our innovative IT-focused environment. In this role, you will guide patients through care pathways, coordinate services, and support adoption of online portals, telehealth, and mobile health tools.
