W-2 – Part Time – 2 days per week (non consecutive days) – you pick the days out of Monday – Friday

** Job Summary Telos Health Systems is seeking a
** Psychotherapist (LMHC, LMFT, LCSW, or Licensed Psychologist – PsyD/Ph.D.**)
**** to provide mental health evaluations and psychotherapy services to patients in Skilled Nursing Facilities and Assisted Living Facilities.

This role offers flexibility, clinical autonomy, and strong administrative and leadership support.##

Key Responsibilities
* Conduct comprehensive mental health assessments and diagnostic evaluations.
* Provide individual psychotherapy and ongoing treatment services.
* Develop, implement, and update individualized treatment plans in collaboration with a multidisciplinary care team.
* Maintain accurate, timely, and compliant clinical documentation.
* Coordinate care with facility staff, medical providers, and other healthcare professionals.
* Adhere to all federal, state, and organizational standards, policies, and regulations.## Qualifications### Required
* Active Florida license as one of the following:  + Licensed Mental Health Counselor (LMHC)  + Licensed Marriage and Family Therapist (LMFT)  + Licensed Clinical Social Worker (LCSW)  + Licensed Psychologist – PsyD/Ph.D.
* Master’s degree (LMHC/LMFT) or Doctoral degree (Psychologist) in a relevant mental health field.
* Ability to work independently while collaborating effectively within a clinical team.
* Strong communication and clinical documentation skills.
* Per AHCA requirements, employment for this position requires a Level II background check. Click  for more information.### Preferred
* Experience working with geriatric populations.
* Experience in Skilled Nursing Facilities, Assisted Living, or long-term care settings.
