Duties and Responsibilities:

1. Essential duties and responsibilities include the following and other duties may be assigned:
2. Assists client to dress, bathe, and groom self.
3. Prepares and serves food for client following special prescribed diets.
4. Assists clients into and out of bed, automobile, or wheelchair, to lavatory, and up and down stairs.
5. Reminds client to take prescribed medications as directed by physician or home care nurse.
6. Provides socialization for clients with activities such as reading aloud, playing cards, or other games.
7. Maintains records of services performed and of apparent condition of client, and reports any changes in condition to Case Manager and family.
8. Changes bed linens, maintains client's laundry, and performs light cleaning of client's quarters.
9. Accompanies ambulatory clients outside home, serving as guide, companion, and aide.
10. Assists client with prescribed exercises to increase range of motion. Massages client and applies preparations and treatments such as liniment or alcohol rubs and heat lamp stimulation.
11. Performs variety of miscellaneous duties as needed such as obtaining household supplies and running errands.

Education/

Experience:

1. High school diploma or general education degree (GED) and at least six (6) months related.experience and/or training; or equivalent combination of education and experience.
CNA License.
CPR and First Aids Certification.
Language

Skills:

1. Ability to read and comprehend simple instructions, short correspondence, and memos.
2. Ability to write simple correspondence.
3. Ability to effectively present information in one-on-one and small group situations
