Member needs hands-on caregiving support due to very limited walking and lower-limb weakness
. Caregiver must be comfortable providing steadying assistance and close supervision during mobility because the member is a fall risk with balance challenges and may roll ankles easily
. Ideal caregiver is patient, safety-focused, and confident assisting with daily routines while encouraging safe independence. Experience with mobility support, transfers, and monitoring for changes in condition is strongly preferred.
Home environment note: member lives with a roommate. Member is a non-
smoker
. No pets in the home.
Required Duties (Texas STAR+PLUS Caregiver Requirements)
Duties will be based on the service plan and may include:
- Mobility & Safety Support: standby assist with walking, balance support, fall-prevention awareness, and safe supervision during movement
- Transfers/Positioning: assist with safe transfers (as needed) and ensure safe positioning to reduce risk of falls/injury
- Personal Care (as authorized): bathing, grooming, dressing, toileting, and hygiene support
- Meal Support: meal prep, light cooking, and assistance with eating as needed
- Light Housekeeping: tidying, dishes, wiping surfaces, sweeping/mopping as appropriate, trash removal, and laundry/linen changes
- Medication Assistance: reminders/assistance as permitted (no medication administration beyond program guidelines)
- Observation & Reporting: monitor for changes in condition, document tasks completed, and report concerns to the agency promptly
Driving
Note:
Shopping/errands were not specified on the intake. If later added to the care plan, caregiver may need reliable transportation.
