* What You’ll Do:
*** Assesses and identify the client’s holistic health care needs/concerns as part of a family and community.
* Assesses social/emotional factors that are related to the patient’s illness, need for care, response to treatment, and adjustment to care.
* Develops a plan of care under physician orders with realistic/measurable outcomes in collaboration with the client/family and health care team providers according to Organization protocols.
* Implement patient care plan activities based on treatment goals towards independent living for the patient/family.
* Serve as resource for staff education & training, participate in case conferences as needed.
** What You’ll Bring:
*** Master’s degree from a school of social work accredited by the council on social work education.
* Maine Board of Social Work Licensure- MSW or LCSW required
* Current CPR card required.
* One or more years of experience as a master’s level social worker in a health care setting.
* A valid Drivers License and reliable car with insurance.
