Description
Job Purpose and Objectives:
Responsible for providing direct nursing care and public health services to individuals, families and the community for Montgomery County.

Position Requirements :
- High school diploma or other equivalent
- Bachelor’s degree in Nursing preferred
- Three years of nursing experience
- Two years in grant management or public health program coordination preferred
Certifications / Licenses:
- Registered Nurse in the State of Kansas
Skills/Knowledge/Abilities:
- Knowledge of public health and community nursing practices
- Displays excellent grant reporting and compliance, budgeting and project management skills
- Proficient in Microsoft Office Suite and public health software
- Excellent time management skills and ability to multi-task and prioritize work
- Ability to fulfill all duties with minimal supervision and to work independently
- Assess situations rapidly and make logical decisions in a timely manner
- Analyze facts and exercise sound judgment
- Ability to effectively manage projects and multiple priorities simultaneously
- Strong written and verbal communication skills
- Problem solving and critical thinking skills

Administers immunizations, medications and treatments as prescribedConducts health assessments, screenings and physical examsProvides nursing care in communicable disease control, maternal/child health, family planning and chronic disease managementCollects and manages lab specimens, interpret results and follows up with patients as neededEducates patients and families on health conditions, treatment plans and preventive careOffers counseling and referrals for services such as nutrition, mental health, substance use and family supportParticipates in community health programs, health fairs and emergency response effortsProvides home visits as part of health promotion activitiesMaintains accurate and confidential patient records using electronic health systemsDocuments services in compliance with departmental, state and federal guidelinesReports communicable diseases and other public health concerns as required by lawResearches available nursing and public health-related grants at the federal, state and private levelsCollaborates with leadership to assess funding needs and prepare grant applications and proposalsDevelops timelines, budgets and objectives for grant projectsMonitors grant expenditures and ensures compliance with all reporting and fiscal requirementsOversees implementation of grant-funded programs and projectsCoordinates with internal teams and external partners to achieve grant deliverablesEnsures project goals align with the county’s public health priorities and strategic planMaintains accurate and up-to-date records of grant activity and performance metricsPrepares and submits required reports to funding agencies on timeAssists with internal audits and evaluations of grant-funded programsServes as liaison between nursing/public health staff and grant funding agenciesTrains and supports staff on grant-specific procedures and complianceRepresents the department at local, regional and state-level meetings as neededEnforces and maintains compliance with all federal, state and local laws and ordinancesPerforms other duties as assigned or deemed necessaryComplies with all organizational and departmental policies and proceduresOperates all job-related equipment, machinery, tools and other aids as required or neededProtects and maintains any confidential information you have access to, whether oral, written, or electronic
