** to join our healthcare team. In this role, you will provide psychosocial support, counseling, care coordination, and community resource referrals to patients and families navigating complex medical conditions.

* Key Responsibilities:

*** Assess patients’ social, emotional, and environmental needs.
* Develop and implement individualized care plans.
* Provide counseling, crisis intervention, and patient advocacy.
* Collaborate with physicians, nurses, and interdisciplinary team members.
* Connect patients and families with community resources and support services.
* Maintain accurate documentation in compliance with organizational and regulatory guidelines.

* Qualifications:

*** Degree in Social Work (or related field) from an accredited program.
* Current social work licensure or eligibility for licensure, as required. LSX Minimum.
* Experience in healthcare, home health, hospice, or community-based services preferred.
* Strong communication, assessment, and care coordination skills.
