Description:

This position involves providing care predominantly to geriatric patients.

The schedule is outpatient, specifically in a clinic setting, Monday to Friday.

We are considering candidates for full-time or part-time (3-4 days per week).

A minimum of 2 weeks per month is required, ideally full-time availability.

There are no on-call duties included in this role.

The provider will have the opportunity to work with an interdisciplinary team of 11 professionals.

Patient visits may occur in-clinic or in the community, including home visits.

For off-site visits, the provider will travel to patients' residences, which may include assisted living facilities or personal homes.

Typically, a nurse from our team will accompany the provider during these visits.

The patient management will be primarily handled through an electronic medical record system.

Patient encounters per shift are typically between 4-6.

Please be prepared to uphold a high standard of care for patients aged 55 and older.

This role is essential in providing comprehensive and inclusive care for the elderly population.
