Therapy - SLP

Experience:

Minimum of 1 year of experience in Speech and Language Pathology Long Term Care or Rehabilitation experience

License:

Massachusetts State License

Certifications:

CCC certified Speech and Language Pathologist

Must-Have:

Experience with Level II clinical practice

Description:

Effectively screens, evaluates, and rehabilitates patients with communication, cognitive, or swallowing disorders.

Develops appropriate plans of care following regulatory and clinical practice standards.

Utilizes standardized assessments and evidence-based practices to support clinical interventions.

Assesses patient progress and adjusts treatments according to regulatory and clinical practice requirements.

Evaluates treatment outcomes for effectiveness.

Modifies services based on patient evaluations and makes referrals in relation to outcomes.

Provides consultation and counseling to patients, families, caregivers, and other service providers regarding speech disorders.

Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans.

Provides comprehensive discharge summaries of services in compliance with regulatory and clinical requirements.

Documents all required information in the electronic documentation system and maintains patient records according to policy.

Accurately records treatment minutes for services provided per regulatory and clinical requirements.

Participates in required meetings, including patient care conferences, utilization review meetings, and rehabilitation meetings.

Participates in facility and company-required training, in-services, and conferences.

Obtains necessary durable medical equipment or supplies in accordance with regulatory and clinical practice requirements.

Supervises and supports CFY or Student Speech-Language Pathologists.

Adheres to professional practice standards, codes of conduct, and ethics as a Speech and Language Pathologist.
