We are seeking a Behavioral Health Therapist III (LCSW) who is passionate about making a difference in the lives of children, adolescents, adults, and families. What You'll DoAs a Behavioral Health Therapist III, you will provide comprehensive behavioral health services in a collaborative care environment where medical and behavioral health professionals work together to support the whole person.
Responsibilities include:
- Conducting behavioral health assessments and establishing diagnoses
- Developing individualized treatment plans with measurable goals
- Providing counseling and therapeutic interventions to children, adolescents, adults, and families
- Supporting patients in achieving self-management and wellness goals
- Responding to behavioral health crises and coordinating appropriate interventions
- Collaborating with primary care providers and interdisciplinary team members
- Assisting patients in managing chronic medical and behavioral health conditions
- Referring patients to specialized services when needed
- Providing education and support to patients, families, and staff
- Maintaining timely, accurate clinical documentation
- Ensuring compliance with confidentiality and regulatory requirements
We are looking for someone who combines strong clinical skills with genuine compassion and a commitment to serving others.
Required Qualifications- Master's Degree in Social Work
- Current Kentucky Licensed Clinical Social Worker (LCSW, LMFT, LPCC) credential
- Minimum of two years of direct clinical experience
- Current CPR certification or ability to obtain and maintain certification
- Commitment to continuing education and professional development
- Experience in integrated primary care or community health settings
- Experience working with underserved or diverse populations
- Knowledge of evidence-based behavioral health practices
- Bilingual English/Spanish skills are welcomed
