The best career is right here. St. Elizabeth Healthcare is hiring a Hospice Care Coordinator for the Hospice unit located at the Edgewood, Kentucky location. Shift &
Hours:
1st shift; PRN (less than 20 hours weekly).
- Establish and maintain agency contacts with area health facilities and community organizations.
- Facilitate client transfer between the facilities and the hospice home care and inpatient unit to ensure continuity of care.
- Provide education to the community on the Hospice Philosophy of Care.
- Demonstrate respect, dignity, kindness, and empathy in each encounter with all patients, families, visitors, and other employees regardless of cultural background.

- Education, Credentials, Licenses:
Graduate of an accredited school of nursing; licensed to practice in the state where work is being performed. If eligible, must have a Compact (multi-state) License in their primary state of residence. Meets contact hour requirements for licensure, including all state-required courses. Valid Driver’s license. - Specialized Knowledge:
Demonstrates advanced assessment skills appropriate for home care environment, ability to function independently; knowledge of community resources. - Kind and Length of
Experience:
2-3 years of RN experience in a hospital or care facility setting. - FLSA Status:
