The Medical Case Manager will work within an interdisciplinary team to deliver comprehensive case management for adults in the clinic program, coordinating services and ensuring adherence to HRSA/Ryan White guidelines.
The role requires strong social work or nursing foundations, excellent communication, and the ability to document care plans and referrals in an EMR system.
