Salary Range: $63,000.00 To $66,000.00 Annually
The Adult Health Home Program Manager
is an integral part of the Care Management team.
The Adult Health Home Program Manager is responsible for supervision of staff and appropriate program operations, provides technical assistance, helps facilitate decisions, coordinates meetings and conducts orientations of new staff.
The Adult Health Home Program Manager Acts as a liaison and resource to clients and staff. This position manages agency Adult Care Coordinators.
The Adult Health Home Program Manager does so by performing the following:
Duties & Responsibilities:
- Enhance the programs capacity to provide a high standard of practice, by fostering good working relationships and providing consultation, technical assistance and case management support to Care Management Supervisors and Adult Care Coordinators, including coverage during the absence of Care Management Supervisors and/or Adult Care Coordinators.
- Assists with overall planning and direction of the Adult Health Home Program.
- Single Point of Contact for informational updates, policy and procedures and every day functions of The Adult Health Home Program.
- Responsible for updates, policy and procedures, quality improvement, electronic health records and billing of multiple Health Homes that ICAN works with in the Adult Health Home Program.
- Responsible for interviews, hiring and providing proper training of department staff.
- Provides ongoing training to support developmental needs of staff.
- Review and compiles appropriate records and reports as required for the program.
- Implements continuous quality improvement plan.
- Facilitates and conducts regular program meetings.
- Ensures compliance with documentation standards, program and agency policies within assigned work unit consistent with the agency’s mission and vision.
- Networks with community organizations and works to develop community contacts to promote the understanding of program services.
- Participates in the development and maintenance of necessary documentation and reports according to program regulations.
- Establishes a work environment that creates positive communication between employees and assimilates new employees to the Agency’s culture, values and mission.
Education/
Experience:
Bachelor’s Degree in Human Services, Social Services, Psychology or related field with at least 3 years experience; and a minimum of 2 years experience as a supervisor.
