This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Director, Community Health Operations based in the United States.

This is a senior operational leadership role responsible for scaling community-based healthcare delivery across multiple markets and health plan partnerships. You will lead distributed frontline teams and ensure community health workers, nurses, and care coordinators operate as one integrated care team. The role combines workforce planning, operational performance, compliance, financial accountability, and care-model transformation. You will translate enrollment forecasts and care model requirements into practical deployment strategies across counties.

You will also represent community operations in health plan discussions and ensure field execution matches contractual and regulatory expectations. This is an opportunity to shape a growing, technology-enabled care model while improving access and outcomes for underserved populations.
