Care Med Health Solutions is a facility‑based team of medical providers who provide medical services to patients living in long‑term care facilities. We strive to give quality care to residents while working together with the facility to carry out the best care possible. While an interdisciplinary care team provides medical care to patients in long‑term care facilities, you will have the opportunity to impact the life and wellbeing of the elderly population positively.

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long‑term care facilities.

Wound NP Role and Responsibilities
  • Provide wound care services for long‑term care patients residing in skilled nursing facilities.
  • Prescribe appropriate medications, including controlled substances, in accordance with KY Law.
  • Order, review, and follow up with treatments and diagnostic testing.
  • Communicate with nursing home staff on a regular basis and as needed to discuss changes in condition and/or the care plan.
  • Collaborate with the primary care physician.
Qualifications and Education Requirements
  • Current, unrestricted NP license in the state of Kentucky.
  • Board Certification.
  • DEA licensure (Optional).
  • Clinical background in adult, family, or geriatric specialties.
  • Clinical background in wound care, including chronic, debilitating, stalled, or infected wounds.
  • Comfortable diagnosing and treating diabetic, arterial, venous, and mixed‑etiology wounds or Kennedy ulcers.
  • Comfortable staging pressure injuries according to the NPIAP.
  • Knowledge of wound care best practices and advanced wound care products (collagen, alginate, foam, silver, etc.).
  • Experience with wound vacs and other DME, such as pressure‑relieving/reducing devices.
  • Strong preference for wound certification or openness to obtaining it.
  • Experience working in a long‑term care setting.
  • Ability to work with an EMR.
  • Ability to work within an interdisciplinary care team to coordinate patient care.
