As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home.
While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.
Monday-Friday, 8:00am-5:00pm
Position SummaryThe Referral Specialist manages incoming referrals with efficiency and urgency to ensure timely coordination of care for patients, families, and referral partners. This role supports seamless navigation through the admissions process, facilitates clear communication among care teams, and provides consistent status updates to referral sources. The Referral Specialist is instrumental in delivering a responsive, patient-centered experience while supporting access to hospice services through Bluegrass Hospice Care.
PositionQualifications
- Graduate of an accredited Practical Nursing or Vocational Nursing program.
- Current, active Licensed Practical Nurse (LPN) licensure in good standing with the State Board of Nursing.
- Minimum of two (2) years of nursing experience required; experience in hospice, community health, medical-surgical, or long-term care settings preferred.
- Demonstrates knowledge of and adherence to accepted nursing standards, professional practices, and ethical guidelines.
- Strong assessment and critical thinking skills, with the ability to identify patient, caregiver, and referral source needs, concerns, and barriers.
- Excellent verbal and written communication skills, including the ability to effectively educate and present information to patients, families, and healthcare professionals.
- Proven ability to build and maintain collaborative working relationships with interdisciplinary teams and external healthcare partners.
- Understanding of hospice philosophy, including the physical, emotional, and psychosocial needs of terminally ill patients and their families.
- Ability to work independently while managing multiple priorities in a fast-paced environment.
- Compliance with all Bluegrass Care Navigators (BCN) policies, including Employee Health, infection control, and vaccination requirements.
- Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.
