Salary Range: $25.44 to $32.74 Hourly

Shift Differential up to $4.50 per hour

Centre Care is a new, state of the art, Rehabilitation and Wellness Facility, dedicated to our community and providing advancing medical care and services through a resident centered care approach for individuals in need of recovery after a hospital stay, sudden onset of illness, injury, effects of surgery or for those in need of care related to chronic disease management. • Receiving and giving reports, providing thorough information regarding resident condition and needs.
• Assuring that CNA’s are aware of any specific appointments or therapies of residents.
• Performing treatments correctly, following Doctor’s orders and Centre Crest Policy and Procedures.
• Providing care for ill residents and appropriate communication to supervisory staff.
• Performing intermittent catheterizations and, as ordered, inserting Foley catheters and operating suction machines.
• Administering medication correctly following Centre Crest Policy and Procedure and completing correct charting.
• Supervising and assisting with training of new CNA’s.
• Assuring daily that residents are cared for correctly by all CNA’s.
• Serving meals to residents and assisting or feeding those who need help.
• Assisting with admission of new residents: collecting required data, including resident vital signs.
• Performing correct, accurate, appropriate charting and remaining aware of specific documentation required by State and documenting accordingly.
• Collecting information to assist in preparing MDS and develop Care Plans.
• Managing time to allow for documentation needed, care tracker charting, and documentation of meds and treatments.
• Transcribing physician orders per facility protocols.
• Holding knowledge of State and Federal regulations and Centre Crest policy.
• Managing stress tolerance, pressure, and anxiety.

• Education:
  
  High School Diploma or its equivalent is preferred.
• License to practice as a Licensed Practical Nurse issued by the Commonwealth of Pennsylvania.
• Experience:
  
  1 year experience in a healthcare setting.
