ResponsibilitiesDuring a visit up to one hour, providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides insight difficult to capture during routine office visits.
- Conduct adult/geriatric assessments, including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow‑up, and clear documentation per Matrix guidelines.
- Administer and order point‑of‑care and lab‑based testing as needed.
- Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
- Collaborate with Primary Care Physician on patient education and follow‑up as necessary.
- Provide services at the member’s place of residence, including homes or skilled nursing facilities.
- Flexibility: Visits schedule based on your availability from Sunday to Saturday, 8 am‑7 pm. Work during day, evening, or weekend as fits your schedule.
- Competitive Pay: Earn income based on number of assessments completed. Monthly earnings range: $1,800 – $11,500 for 20 – 100 visits, per assessment rate $90 – 115, depending on state location.
- Benefits: Mileage reimbursement, 401(k), employee referral bonuses, and limited voluntary benefits.
- Education and Support: Comprehensive onboarding training, a preceptor, 1:1 feedback, and free access to continuing education.
- Additional Licensure: Support for obtaining licensure and credentialing in neighboring states as needed.
- Full‑time Opportunities: PRN providers may transition to full‑time roles with a sign‑on bonus.
- Master’s Degree or equivalent experience and successful NP licensure.
- Current NP licensure in state of practice with prescription authority, or ability to obtain prescriptive authority.
- Board certification (AANP, AACN, or ANCC) in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
- Current BLS, ACLS or CPR certification.
- 1 year of NP experience preferred; new graduates encouraged to apply.
- Valid state driver’s license and required automobile insurance for the state of residence.
- Strong computer skills and familiarity with employee health/medical record software.
- Excellent verbal and written communication skills.
