Key Responsibilities Administer medications and treatments as prescribed

Monitor and document resident conditions and report changes promptly

Assist residents with daily care needs and respond to concerns professionally

Ensure compliance with federal, state, and facility regulations

Communicate effectively with residents, families, and interdisciplinary staff

Respond to emergencies using sound nursing judgment

Promote a supportive, positive, and team-oriented environment

Qualifications

Current LPN license in the State of Kentucky Strong clinical and assessment abilities

Ability to work independently and collaboratively

Excellent communication and organizational skills

Compassionate, resident‑focused approach to care
