We are seeking a dedicated and detail-oriented LPN or RN to join our team as an MDS Assistant
. No prior MDS experience is required, we will provide education and training for the right candidate.
In this role, you will assist the nursing administration team with MDS-related responsibilities, support clinical operations, and participate in the on‑call rotation. This is an excellent opportunity for a LPN/RN looking to expand their knowledge and develop new skills in long‑term care.

Core Responsibilities- Consult and collaborate with healthcare team members to assess, plan, implement, and evaluate resident care plans.
- Prepare, administer, and document prescribed medications in accordance with physician orders and facility policies.
- Monitor residents for adverse reactions to medications or treatments and initiate appropriate interventions when necessary.
- Record residents' medical information, assessments, and vital signs accurately and timely.
- Monitor and adjust specialized equipment as needed and interpret electronic displays.
- Provide bedside nursing care to residents with a variety of medical needs.
- Perform clinical procedures, including dressing changes, catheter insertions, and IV initiation as permitted by scope of practice.
- Prepare equipment and assist physicians during examinations and treatments.
- Respond appropriately to emergency and life‑saving situations in accordance with nursing standards and established protocols.
- Participate in discharge planning and care coordination activities.
- Utilize effective time management and organizational skills to prioritize responsibilities.
- Complete all required documentation accurately, concisely, and within established time frames using appropriate forms and systems.
- Assist nursing administration with MDS‑related processes and duties as training progresses.
- Participate in the nursing administration on‑call rotation.
- Perform other duties as assigned to support resident care and facility operations.
- Current RN or LPN license in the state.
- Minimum of one (1) year of experience in a long‑term care setting preferred.
- Strong communication, organizational, and critical thinking skills.
- Willingness to learn MDS processes and successfully complete provided training.
