MDS Coordinator (Licensed Nurse) Tradewater Health & Rehabilitation part of Concord Health Systems in Dawson Springs KY

Tradewater Health & Rehabilitation is a small, family‑owned nursing home committed to meaningful change, higher quality standards, and a renewed focus on honoring the fact that our building is the home of our residents. We are seeking an experienced MDS Coordinator who is a licensed nurse (RN or LPN) and is passionate about accuracy, accountability, and resident‑centered care.

This role is essential to ensuring that every resident receives the quality of care they deserve — and that our documentation reflects the respect we have for their home.

Key Responsibilities
  • Lead the MDS/RAI process with accuracy, timeliness, and regulatory compliance
  • Complete assessments that truly reflect each resident’s needs, strengths, and goals
  • Collaborate with the interdisciplinary team to develop care plans that honor residents’ preferences and dignity
  • Support quality improvement efforts and help drive positive change throughout the facility
  • Monitor quality measures and ensure documentation supports high‑quality outcomes
  • Participate in care plan meetings and communicate clearly with staff and leadership
  • Promote a culture where documentation, accountability, and resident respect are non‑negotiable
Qualifications
  • Active Kentucky RN or LPN license (required)
  • Previous MDS/RAI experience (required)
  • Strong understanding of PDPM, care planning, and regulatory requirements
  • Commitment to quality, accuracy, and continuous improvement
  • Excellent communication and teamwork skills
  • Passion for supporting a resident‑first culture in a home‑like environment
