Care Med Health Solutions is a facility-based team of medical providers who deliver medical services to residents living in long-term care facilities. We strive to provide quality care and collaborate with facilities to ensure the best care possible. The interdisciplinary care team supports medical care for residents in long-term care facilities, and you will have the opportunity to positively impact the life and wellbeing of the elderly population.
We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.
WOUND NPROLE AND RESPONSIBILITIES
- Provide wound care services for long-term care patients residing in skilled nursing facilities.
- Prescribe appropriate medications, including controlled substances in accordance with KY Law.
- Order, review, and/or follow up with treatments and diagnostic testing.
- Communicate with the nursing home staff regularly and as needed to discuss changes in condition.
- Discuss changes in condition and/or the care plan.
- Collaborate with the primary care physician.
- Current, unrestricted NP license in the state of Kentucky.
- Board Certification.
- DEA licensure (Optional).
- Clinical background in adult, family, or geriatric specialties.
- Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds
- Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
- Must be comfortable staging pressure injuries according to the NPIAP.
- Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
- Must have experience with wound VACs and other DME, such as pressure-relieving/reducing devices.
- Strong preference to having wound certification or being open to getting certified.
- Experience working in a long-term care setting.
- Ability to work with an EMR.
- Ability to work within an interdisciplinary care team to coordinate patient care.
