Registered Nurse - After-Hours Hospice

As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home.

While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

Schedule: Monday-Thursday nights, 5:00pm-8:00am

Location: Whitley, Laurel, Clay, and Knox Counties

Incentive: $4.00 per hour on-call shift differential pay, in addition to base pay

Position Summary:

The After-Hours Hospice RNdeliverscompassionate, patient-centered care for individuals receivingcomprehensive health care services.

Thiscritical role ensures continuity of care by responding to patient needs, managing symptoms, providing emotional support, and offering clinical guidance to patients and families outside of regular business hours.

The After-Hours Hospice RN willwork independentlyintheir home setting and will perform on-site visitsto the Hospice patient’s home care settingor wherever the patient calls homewhen needed.

Home care settingsmayconsistofa private residence, skilled nursing facility, personal care home, and/or other interim livingarrangements. A Registered Nurse in this position will havestrongcommunicationskills, the competence to work autonomously, professionalism to work within an interdisciplinary team, and the ability to provide care with empathy. Position

Qualifications:

• Graduate of an accredited school of registered nursing.
• Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or through a participating Compact State Board of Nursing.
• Minimum of one (1) year of general nursing experience preferred; new graduate nurses will be considered.
• Demonstrates strong clinical nursing skills and the ability to maintain professionalism during emotionally challenging situations.
• Provides compassionate, patient-centered care while serving diverse populations with respect and cultural sensitivity.
• Adheres to accepted professional, ethical, and legal standards of nursing practice.
• Demonstrates understanding of and commitment to hospice philosophy and the principles of death and dying.
• Maintains current CPR certification.
• Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.
