Licensed Practical Nurse

Schedule:

Monday-Friday, 8am to 5pm

Pay: $26.66 to $33.28 per year

Sign-On Bonus: $6,000

JOB SUMMARY

The Licensed Practical Nurse (LPN) is a member of the PACE interdisciplinary team; provides nursing care to the participant and assists the Registered Nurse with the implementation of the Plan of Care. The LPN provides care for the participant in a manner that promotes an interdisciplinary team approach, respects participant's goals for care, meets Medicare/Medicaid conditions of participation and accreditation requirements and reflects the mission and philosophy of TRU PACE.

Participates in all areas of clinical services.

EDUCATION AND QUALIFICATIONS
  • Must possess a current LPN license in the state of Colorado
  • Minimum one year LPN experience - hospice or home health experience preferred
  • Experience working with frail/elderly preferred
  • Experience working with individuals with mental health needs desirable
  • Bilingual in Spanish preferred
  • Must successfully pass agency required background checks and drug screening
  • IV certification and phlebotomy skills preferred
ESSENTIAL DUTIES
  • Collects data regarding all body systems, signs and symptoms, deviations, acuity, vital signs, and responses in collaboration with the Registered Nurse and primary care physician,
  • Incorporates participant preferences and goals within the plan of care
  • Supports and respects the participant and caregiver's unique diversity, including spiritual and cultural beliefs.
  • Completes documentation in an accurate and timely manner as per agency standard and in compliance with regulatory standards
  • Provides participant and caregiver education as appropriate; areas of education include, but are not limited to personal care, medications, procedures, safety, diet and nutrition, and health and wellness through the lifespan
