Journey Care Hospice seeks an experienced Hospice RN Case Manager to join our growing team in Barrington, IL. Shift is Monday–Friday 8:30 a.m.–5:00 p.m. One week on call every 12 weeks.
You will manage a patient and family caseload, coordinate interdisciplinary care, and support families through the end-of-life process. You will assess needs, develop care plans, and communicate with physicians and team members to ensure compassionate, quality hospice care.
