Suncoast Hospice, part of Empath Health, is seeking a compassionate RN Case Manager for a Monday–Friday 8:00 a.m. to 5:00 p.m. schedule in South Pinellas County. You will coordinate end-of-life care, assess patient needs, and collaborate with the Interdisciplinary Group to implement individualized care plans.
The role requires an approved nursing degree, an active Florida RN license, CPR certification, and hospice experience. CHPN is encouraged within three years.
