Trustbridge Hospice is seeking a caring RN Hospice Case Manager to join our Broward County team. This role involves coordinating patient care with the primary physician and RN, identifying needs, and delivering individualized care plans to enhance comfort and quality of life.
You will work PRN from 8:30am to 5pm with weekend availability. Florida RN licensure, BLS, and hospice knowledge are essential, with BSN and hospice certification preferred.
