As a Home Health LPN, a vital part of the patient s care team is provided by the employee, offering skilled nursing care and support. This includes tasks such as:

• Assisting with wound care and personal care
• Educating patients and families
• Maintaining medical records
• Collaborating with other healthcare professionals. Also, provide emotional support and companionship, building genuine connections with patients.

• A minimum of one year of LPN experience (Hospice experience is preferred but not required).
• Excellent communication, interpersonal, and organizational skills.
• Ability to work independently and as part of a collaborative team.
• Compassion, empathy, and emotional resilience to provide comfort and support during end-of-life situations.
