PT (PM/ 2nd Shift) 24hr/week
$2,500 SIGN ON BONUS
Maplewood is currently interviewing LPNs that can create a healing environment focused on the physical and emotional well-being of all our residents. An LPNs responsibilities include daily resident care, administering medications and treatments, monitoring health changes, and collaborating with physicians and RNs to ensure a safe, compassionate environment for our Long‑term Residents and Short‑Term Residents that are here for rehab. LPNs supervise certified nursing assistants (CNAs) to ensure resident safety and health.
Alternating weekends required.
Job Duties
- Resident Care:
Assist residents with daily living needs and check vital signs regularly. - Medication Administration:
Give prescribed oral, topical, and injectable medications safely. - Treatment Delivery:
Perform wound care, dressing changes, and other clinical treatments. - Health Monitoring:
Observe resident status and report any sudden changes to the medical team. - Documentation:
Record accurate clinical notes and care updates in electronic health records. - Team Support:
Guide certified nursing assistants (CNAs) and help coordinate daily shift tasks.

- License:
Active and unencumbered LPN license in the state of Wisconsin. - Experience:
Previous clinical experience in long‑term care or geriatrics is preferred. - Skills:
Critical thinking skills, crisis management, multi-task oriented, and knowledge of long‑term care regulations. Strong communication, patience, and basic computer skills for charting.
- Competitive pay starting at $27.70/hour plus differentials
