Garden Gate Health Care Facility is looking to add an LPN Unit Coordinator of Subacute to its nursing leadership team. In this role, you'll oversee the daily clinical operations of an assigned nursing unit, ensuring high-quality resident care, regulatory compliance, and effective coordination among residents, families, providers, and interdisciplinary team members. RESPONSIBILITIES:- Oversee the clinical care and daily operations of an assigned nursing unit
- Supervise nursing staff, nursing assistants, and unit support staff while providing leadership and guidance
- Monitor resident conditions, review assessments, and ensure care plans are updated and implemented appropriately
- Coordinate communication with physicians, nurse practitioners, physician assistants, residents, and families
- Review laboratory results, provider orders, incident reports, and changes in resident condition
- Participate in care plan meetings, QAPI activities, family meetings, and interdisciplinary team discussions
- Assist with admissions, discharges, transfers, and coordination of outside appointments
- Ensure unit supplies, documentation, and resident records are accurate and current
- Support staff education, orientation, infection prevention efforts, and quality improvement initiatives
- Address resident, family, and staff concerns while promoting a positive care environment
- Perform additional duties as assigned
- High School Diploma or equivalent required;
Associate's Degree preferred - Valid NYS Licensed Practical Nurse (LPN) License required
- Minimum 1 year of LPN experience in a long-term care setting required
- 3-5 years of long-term care experience with supervisory experience preferred
- Strong knowledge of resident care planning, clinical documentation, and long-term care regulations
- Leadership experience with the ability to supervise, coach, and mentor staff
- Excellent communication, organizational, and problem-solving skills
- Ability to coordinate care across multiple disciplines and manage competing priorities
