Location: Northern
Father Baker Manor Sub Acute in Orchard Park, NY seeks a Coordinator MDS RN for part-time hours. The role coordinates assessments, supports clinical staff, and helps ensure high-quality resident care. Work is scheduled days from 8A-4P with benefits included.
Requirements include NY RN licensure, nursing degree, RAC-CT certification within one year, and maintenance of RAC-CT (MDS) certification. One year of experience in a residential health care setting is preferred.
