Location: Hartford
Signature HealthCARE is seeking a Licensed Practical Nurse to join our Healthcare & Medical Services team. In this role, you will provide direct nursing care, administer medications, monitor resident conditions, and collaborate with an interdisciplinary team to support individualized care plans. You'll deliver compassionate, resident-centered care in a long-term and post-acute setting, ensuring dignity, safety, and comfort. Responsibilities
- Provide direct bedside nursing care to residents and patients in accordance with the LPN scope of practice and facility policies.
- Monitor, record, and report vital signs and changes in resident condition to the supervising RN or provider.
- Administer medications and treatments as ordered, ensuring safe medication practices and accurate documentation.
- Perform basic clinical procedures such as wound care, dressing changes, catheter care, and tube feeding assistance as permitted by state regulations.
- Contribute to the development and implementation of individualized care plans and participate in interdisciplinary care conferences.
- Maintain accurate, timely documentation in the Electronic Health Record (EHR) and other required records.
- Uphold infection control, safety, and quality standards, including proper use of PPE and adherence to facility protocols.
- Educate residents and families on medications, treatments, care plans, and health maintenance, promoting understanding and engagement.
- Collaborate with RNs, CNAs, therapists, physicians, and other staff to coordinate care and support positive clinical outcomes.
- Support admissions, transfers, and discharges to ensure smooth transitions and a positive resident experience.
- Participate in quality improvement and performance initiatives aligned with Signature Health
