Licensed Practical Nurse

Southern Health Partners Inc seeks a dedicated LPN to deliver compassionate, evidence-based care to incarcerated patients in a secure correctional setting. The LPN will perform assessments, administer medications and treatments, support chronic disease management, respond to urgent health needs, and maintain accurate EMR documentation while ensuring regulatory compliance and safety. Responsibilities

• Provide direct nursing care to incarcerated patients, including assessments, treatments, and follow-up
• Administer medications, vaccines, and treatments per provider orders and facility protocols
• Monitor, document, and report changes in patient condition using EMR systems
• Assist providers with sick call, chronic care, and urgent/emergent situations
• Perform wound care, specimen collection, and basic diagnostic tests
• Support compliance with state regulations, infection control, and safety standards
• Collaborate with multidisciplinary correctional and healthcare teams
• Educate patients on health maintenance, medications, and chronic disease management
• Participate in on-call or shift rotation as required by facility needs
