Senior Case Manager

Cardinal Hill Rehabilitation Hospital, part of Encompass Health, seeks a Senior Case Manager to coordinate comprehensive discharge planning and care transitions for adult rehab patients. In this role, you'll manage complex caseloads, conduct psychosocial assessments, collaborate with an interdisciplinary team, and advocate for patient needs. Responsibilities include insurance authorization, resource identification, family education, and community referrals. You will track outcomes, maintain accurate documentation, and support quality, patient-centered care while helping patients regain function and independence in a collaborative, mission-driven environment.

Responsibilities

• Manage a complex caseload of inpatient rehabilitation patients from admission through discharge
• Complete psychosocial assessments and develop individualized discharge and care transition plans
• Coordinate with an interdisciplinary team including physicians, therapists, and nursing staff
• Obtain and manage insurance authorizations and benefits verification
• Educate patients and families on care plans, community resources, and post-discharge expectations
• Arrange home health, equipment, outpatient therapy, and community support services
• Advocate for patient needs and remove barriers to safe, timely discharge
• Maintain accurate, timely documentation in the electronic medical record
• Monitor outcomes, length of stay, and readmission risks; participate in quality initiatives
• Collaborate with leadership to improve case management processes and patient experience
