General PurposeThe LVN performs selected nurse care under the direction of the registered nurse for patients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient/family and referring agency. Ensure staff and other support resources are appropriately utilized and a high level of interdisciplinary team work is maintained through delegation of duties.
Essential Job Functions- Provides skilled nursing care to clients as directed by the registered nurse and physician's plan of care in accordance with agency policies.
- Promotes and maintains client health through teaching and appropriate rehabilitative measures under the guidance of the registered nurse who is assigned to the same case.
- Assists clients in learning appropriate self-care techniques.
- Helps to achieve and maintain continuity of client care by assisting in planning and exchanging information with other health professionals.
- Prepares clinical progress notes and follows client care plans as developed by the registered nurse and according to the agency policy.
- Assist the physician and registered nurse in performing specialized procedures for which competency has been established.
- Prepares the equipment and materials for treatments observing aseptic technique as required.
- Keep abreast of changes and trends affecting nursing care and licensed nurse functions.
- Attends client conferences, meet with the quality improvement reviewer weekly and communicate regularly with the registered nurse.
- Must be capable of performing the job functions of this position with or without accommodations.
- Complies with acceptable professional standards and practice.
- Adhere to all of the Company's policies and procedures.
- Perform other duties as assigned.
- Participates in on-call duties as defined by the on-call policy.
- Ensures that arrangements for equipment and other necessary items and services are available.
- Instructs, supervises, and evaluates home health aide care provided every fourteen (14) days.
- Successful completion of a certified licensed practical/vocational nursing program.
- Licensed to practice as a practical/vocational nurse in the state.
- Minimum of six (6) months to one (1) year of clinical experience.
- Six (6) months to one (1) year of previous home health or hospice experience preferred
- Reliable transportation. Valid and current auto liability insurance
- Must be a licensed driver with an automobile that is insured in accordance with state/or Company requirements and is in good working order.
- Must hold a current and valid CPR certification.
