We are seeking a compassionate, experienced Registered Nurse (RN) to serve as Assistant Director of Nursing (ADON) witha primary focus on Infection Prevention and Quality Assurance ina rural long‑term care community
.

This role is ideal for a nurse leader who values hands‑oncare, strong relationships, and knowing residents and staff by name—not number
.The ADON supports the Director of Nursing in daily operations and provides leadership continuity in the DON’s absence, while ensuring regulatory compliance and safe, resident‑centered care.

• Serveas the facility’s Infection Preventionist
• Conduct and document daily infection control rounds
• Track,trend, and report infection control outcomes
• Ensure compliance with CDC guidelines and state/federal regulations
• Support survey readiness and corrective actions related to infection control
• Communicate findings clearly to leadership and care team members

• Lead Quality Assurance and Performance Improvement (QAPI) efforts
• Complete required nursing audits (daily, weekly, monthly, quarterly)
• Audit:
• Admissions
• Medication and Treatment Records
• Medication and Treatment Carts
• Nurse Aide documentation
• Meal consumption documentation
• Develop practical, achievable action plans focused on improving resident outcomes
• Report findings to the Administrator, DON, and leadership team
• Utilize OIGcritical pathways to support compliance

• Assist with oversight of clinical operations and nursing practices
• Support staff education, competency, and regulatory compliance
• Rotateon the nurse on‑call schedule and assist on the floor as needed
• Work closely with interdisciplinary teams including rehab, dietary, social services, and activities
• Participate in care planning and resident outcome discussions

• Registered Nurse (RN) with active Kentucky license
• Bachelor’sdegree preferred
• Minimum 3years of nursing leadership or supervisory experience preferred
• Long‑termcare, geriatric, or rural healthcare experience preferred
• Strong knowledge of infection control and regulatory requirements
• MDSexperience preferred
• Organized,approachable, and able to make sound independent decisions
• Comfortable working in a hands‑on, team‑oriented rural setting

• Mustbe at least 21 years of age
• Mustbe free of any felony or misdemeanor related to abuse, neglect, or misappropriation
• Abilityto build trust and positive relationships with residents, families, andstaff
