On Call / Triage Registered Nurse Full Time Nights

Regular Full-Time Professionals Fort Pierce, FL, US

Salary Range: $72,000.00 To $76,000.00 Annually

Full time Nights 8pm - 8am

The RN is responsible for the planning and delivery of patient care, providing skilled nursing care and oversight in accordance with Treasure Coast Hospice policies and the Florida Nurse Practice Act, utilizing the nursing process of assessment, planning, intervention, implementation and evaluation. To provide a combination of skilled professional nursing tasks and provide palliative and supportive care to the patient and family unit.

Includes triage and in-person visits as needed.

Qualifications

  • Current and active Florida RN license.
  • Must be Certified Hospice and Palliative Registered Nurse (CHPN) within 12 months.
  • Current and valid Florida driver’s license and automobile insurance.
  • Minimum of two years RN experience with acute care experience, preferably in ER or as a Charge Nurse.
  • Prior experience in oncology, home health, Hospice or palliative care setting is preferred.
  • Must be able to handle sensitive issues appropriately using sound judgment, maintain professional boundaries, and maintain confidentiality.
  • Must be able to effectively plan, organize and prioritize work.
  • Must be self-directed and able to work independently in the field.
  • Basic computer skills and EMR software experience preferred.
  • Must hold a current (BLS) CPR certification.
  • Attendance is a condition of employment.
  • Provide program development strategies for Treasure Coast Hospice , in an organized, routine capacity that will enable the cohesion of team developments. Work collaboratively with other departments, demonstrate and encourage leadership for a comprehensive approach to program development. Inspire staff to live the Treasure Coast Hospice's Mission, to achieve its Vision, and demonstrate our Values and Commitment. Be current in legal and compliance hospice issues.

  • Responsible for upholding Treasure Coast Hospice/Model Standards for patient visits and compliant documentation.
  • Provide comprehensive information to callers or may go out to assist an emergent patient.
  • Clearly understanding and adhering to each Standard.
  • Motivate and lead self with confidence, energy and integrity.
  • Supports service excellence initiatives, by serving internal and external customers in a manner that increases confidence, knowledge, responding with a sense of urgency, efficiently and courteously to needs and concerns.
  • Demonstrates application of Mission, Vision, and Values in all interactions with internal and external customers.
  • Understanding of an ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs.
  • Perform comprehensive assessments to include the physical, emotional, spiritual and psychosocial needs of the patients and family unit.
  • Communicate to the IDT all pertinent information pertaining to the patient and family unit, actively to participate in review and revision of patients’ plan of care.
  • Documentation should include but not limited to variance in patient condition, interventions and outcomes and address progress towards goals set by the patient’s plan of care.
  • Based on verbal assessment determine the needs of the patient and family member.
  • Assigns staff based on the need.
  • Knowledge of commonly used medications used with the Hospice patient.
  • Documentation should include education provided to the family and the patient, including topic, which was taught and their understanding of the education.
  • Makes patient visits to wherever the patient calls home; as indicated by need.
  • Initiate communication with physicians and other staff members to coordinate optimal care and use of resources for the patient/family.
