In this role, you will provide direct clinical support, administer medications and treatments, and monitor health status for individuals of all ages and abilities. Responsibilities
- Provide direct nursing care and health monitoring to individuals in home and community-based settings
- Administer medications and treatments per physician orders and organizational protocols
- Document assessments, interventions, and patient progress accurately and timely in electronic records
- Collaborate with interdisciplinary teams, families, and caregivers to support person-centered care plans
- Educate individuals and families on health conditions, medications, and self-care strategies
- Respond to changes in condition and initiate appropriate nursing interventions or escalations
- Assist with ADLs while promoting independence, dignity, and safety
- Follow infection control, safety, and compliance standards in all care activities
- Participate in ongoing training, competency checks, and professional development
- Support a culture of empathy, inclusion, and collaboration in daily practice
Required Skills
- Vital signs monitoring and basic physical assessment
- Medication administration and reconciliation
- Wound care and basic dressing changes
- Electronic health record (EHR) documentation
- Infection control and standard precautions
- Care planning and coordination with interdisciplinary teams
- Patient and family health education
- Behavioral health and de-escalation techniques
- Time management and case prioritization
- Regulatory and compliance knowledge (HIPAA, state nursing regulations)
