Job Summary :The Home Care Skilled Nurse is responsible for providing skilled nursing services to participants in their homes in accordance with the participant’s plan of care, physician orders, agency policies, and applicable state and federal regulations. The Skilled Nurse performs comprehensive clinical assessments, provides skilled treatments and procedures, monitors changes in condition, identifies abnormalities or potential health concerns, and promptly communicates relevant findings to the interdisciplinary care team.
Reports directly to the HCA Supervisor.
Specifications
Education :
Graduate of an accredited school of nursing and a current RN license
Experience :
At least 1 year as an RN, preferably in either home or community health. Supervisory experience desired. Minimum of 1 years’ experience working with the frail elderly population.
Number and Type of Employees Supervised (optional) :
Licensure, Registry or Certification Required : RN currently licensed in North Carolina; or compact state, able to present credentials which demonstrate ability to meet performance criteria for RN. Valid NC driver's license and vehicle.
Special Training :
Must be able to work effectively in a team environment. Must be able to treat geriatric patients. Only act within the scope of his or her authority to practice. Meet a standardized set of competencies established by Senior Total Life Care and approved by CMS before working independently.
Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact

Key Responsibilities : (

- Conduct thorough head-to-toe physical assessments.
- Assess and monitor vital signs, including blood pressure, pulse, respirations, temperature, oxygen saturation, weight, and other clinically indicated measurements.
- Evaluate the participant’s overall physical condition and identify changes from their baseline.
- Assess cardiovascular, respiratory, neurological, gastrointestinal, genitourinary, musculoskeletal, integumentary, and other body systems as clinically appropriate.
- Monitor chronic health conditions and assess for signs or symptoms of deterioration or complications.
- Identify abnormal assessment findings and promptly report significant changes in the participant’s condition to the appropriate physician, nurse practitioner, interdisciplinary team member, supervisor, or other appropriate healthcare provider.
- Document all assessments, observations, interventions, and communications accurately and timely in accordance with agency policy.
- Using the nursing process, assesses the home care needs of a frail elderly population, and identifies and develops specific plans of care, if applicable.
- Completes in-home initial assessments, re-assessments, and supervisory visits every 90 days.
- Performs in-home visits and nursing tasks as ordered by primary care provider.
- Maintains home care guidelines by writing and updating policies and procedures.
- Maintains safe and clean working environment by designing and implementing procedures, rules, and regulations, calling for assistance from other health care professionals as needed.
- Provides information to patients and health care team by answering questions and requests.
