- Day Shift. In this role, the successful candidate Works collaboratively with the Manager/Assistant Nurse Manager of Hospice Services and is responsible and accountable for establishing and maintaining standards, related to supervision of referrals for Hospice services. Serves as a patient care advocate and role model in the performance of direct and indirect patient care and assumes leadership role and function in absence of the Manager.

• Assists Manager in establishing, monitoring and evaluating standards of patient care focusing on measurable outcomes. Consistently addresses patient careneedsinatimelymanner. Screen inquiries and referrals and processes community requests for Hospice service in accordance with Agency policies and procedures. Completes a comprehensive assessment of patients referred to Hospice services from Hospitals, skilled nursing facilities, rehab facilities and from the community. Responsible for the planning to admit the patient to the Agency.
  
  Determines and ensures client eligibility and authorization to receive requested services and notifies all pertinent staff of approval or denial of requests.
• Participates and/or supervises multidisciplinary process improvement activities for program specific and hospital wide initiatives. Assures compliance with Joint Commission, Department of Health, NYS Department of Social Services and other regulatory agencies, as well as agency policies and procedures. Assists with attainment of specific goals and objectives related to referral management in accordance with organizational and agency strategic goals. Interprets, implements and promotes agency policy;
  
  recommends policy modifications.
• On-Call Duties:
  Collaborates on assessment of patient's clinical status with team members and attempts satisfactory resolution ofpatient clinical issuesinatimelymanner. Addresses patient team member issues following established communication hierarchy. Participates in identification, investigation and resolution ofactual andor potential risk management quality care ethical issues. Holds team members accountable.
• Assumes leadership roleandfunctions intheabsence of Manager. Ensures the provision ofageappropriate and safe care service (population served). Maintain clinical knowledge base appropriate topatient population.
• Responds inatimely manner toorganizationalinformation requests for reports, anddata. Consistently utilizes computer to communicate, document, track and evaluate information.
