# Registered Nurse## Overview Level Experienced Position Type Full Time## Description The Licensed Nurse will assist with the implementation of medical services of Five Horizons Health as a member of an interdisciplinary team of healthcare providers. The Registered Nurse or Licensed Practical Nurse will provide patient care under the direction and supervision of the clinical management team to include but not limited to physicians and Certified Registered Nurse Practitioners of Five Horizons Health Services.
The Licensed Nurse will follow all clinical/agency protocols, policies and procedures to include confidentiality, HIPAA, and OSHA to include adherence to state of Alabama regulations. In state travel to rural clinics is required. Access to Protected Health Information (PHI) and data is required for position performance The Licensed Nurse will follow all clinical protocols, agency policies and procedures to include confidentiality, HIPPA, and OSHA while operating within the State of Alabama regulations.
The Licensed Nurse is willing to work in a diverse environment and is comfortable working with People Living with HIV (PLWH) and LGBTQ+ Community.

* Essential Functions:
*** Act as liaison between patient/client and Providers. Answer all patient/client inquiries for assigned Providers regarding health issues. Serve as advocate for patient/client for both internal and external referrals
* Provide appropriate patient care to include nursing assessment, injections, treatment plans, education, and adherence within RN/LPN scope of practice for the State of Alabama.
* Will follow agency protocol on medication administration per medical provider order, with complete accurate documentation in the EMR
* Provide education, screening and treatment, following standing orders, to clients presenting through the STD, syphilis and hepatitis walk-in clinics.
* Coordinate medical care with other providers as required, schedule patient follow-up internal and external appointments as instructed by Providers
* Initiate, execute and maintain necessary insurance, disability forms, AIDS case reporting, reportable disease morbidity forms and other forms as required
* Perform lab procedures as assigned. Collect, label, and process all specimens including but not limited to urine, stool, semen, sputum, and blood. Ensure that specimens are processed and shipped in accordance with established Five Horizons Health Services laboratory policies and procedures along with OSHA Guidelines Notify providers of alarm values found in lab reports. Provide patient/client with appropriate requisition, information and instruction for ancillary tests and procedures ordered by Providers
* May participate in community programs and outreach services regarding Health Care Services provides by Five Horizons Health Services
* Inspect clinical facilities and act to maintain excellent hygiene and safety (decontaminating equipment, sanitizing clinical surfaces, temperature checks and documentation as required)
* Execute Phone Triage responsibilities: performs a systematic, comprehensive interview of the caller/patient/client to determine the urgency of the patients problem. Accurately documents data for all calls including, but not limited to, patient/client demographic information, chief complaint, recommendations for home treatment, and referrals to primary care provider, alternative health care provider or emergency services. Performs and documents follow-up calls with patients as necessary to help ensure positive outcomes.
* Make referrals to the appropriate level of care within an appropriate period of time. Schedules same-day appointments or follow-up appointments with the patient/client primary care provider or alternative health care provider as necessary
* Foster a supportive and compassionate environment to care for the patients and families of MAO
* Maintain clinical competency through continuing education, recertification (including CPR), and clinical practice.
* Adhere to Howard Brown Human Resources Policies and Procedures.
* Actively participates in fulfillment of organizations mission, vision, and core values. Adhere to OSHA standards in delivery of care to all patients.
* Participate in Quality Improvement process via data collection, evaluation of results, and implementation of recommendations.
* Promote Behavioral Health Integration services through telemedicine support, scheduling with ECG/EKG as ordered or assigned
