Registered Nurse (RN) Care Manager to provide exceptional hospice and palliative care to patients and families throughout
Licking County, Ohio
.

This is an opportunity to build meaningful relationships with patients and families while providing expert clinical care during one of the most important stages of life. As an RN Care Manager, you’ll work alongside physicians, social workers, aides, chaplains, and other members of an interdisciplinary hospice team to ensure every patient receives personalized, compassionate care.

• Full-time | Monday–Friday
• Hours: 8:00 AM–4:30 PM
• Participation in a
  holiday rotation
• Field-based position serving patients in their homes and care facilities throughout the Licking County service region
• Travel throughout the service area is required

As an RN Care Manager, you will coordinate and deliver high-quality hospice care using the nursing process from initial assessment through ongoing evaluation.

Your responsibilities will include:

• Assessing patients and developing individualized plans of care based on their clinical needs and goals.
• Providing skilled nursing care to hospice patients in homes and care facilities.
• Monitoring changes in patient condition and adjusting care plans as needed.
• Managing symptoms and supporting comfort, dignity, and quality of life.
• Educating and supporting patients, families, and caregivers throughout the hospice journey.
• Collaborating with physicians and the interdisciplinary care team to coordinate comprehensive patient care.
• Communicating changes in patient status and advocating for patient and family needs.
• Accurately documenting assessments, interventions, medications, and patient outcomes.
• Following professional nursing standards, hospice regulations, and organizational policies.

• Graduate of an accredited nursing program.
• Current Registered Nurse (RN) license in the State of Ohio
  , in good standing.
• Two years of acute care nursing experience preferred.
• Hospice, palliative care, home health, or community nursing experience is a plus.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred.
• CHPN certification preparation and support are available for eligible employees.
• Current CPR certification.
• Basic computer skills and ability to accurately document patient care.
• Valid driver’s license and safe driving record.
• Reliable transportation and ability to travel throughout the service area.
• Ability to safely drive in varying weather and road conditions.
• Ability to meet state-required automobile insurance requirements.
• Successful completion of a criminal background check and 10-panel drug screening.
