POSITIONDESCRIPTION:
The Registered Nurse (RN) position will support the medical team with various nursing tasks in an excellence-focused outpatient primary care setting. The Registered Nurse will be responsible for facilitating a continuum of patients’ care utilizing advanced knowledge, experience, and skills to ensure the appropriate utilization of resources.

PRIMARY RESPONSIBILITIES- Consistently exhibits behavior and communication skills that reflect SHIFT’s commitment to superior customer service, including quality, care, and concern for every Member.
- Supports effective practice operations by working collaboratively to resolve patient, staff, or operational issues; identifying opportunities for improvement; and ensuring appropriate utilization of medical facilities, equipment, and services by SHIFT medical staff.
- Uses, protects, and discloses SHIFT patients’ protected health information (PHI) in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.
- Collaborates with SHIFT physicians, care team members, external providers, hospitals, clinical vendors, patients, and, when appropriate, patients’ families to manage acute and long‑term medical needs, coordinate treatment plans, monitor progress, and support continuity of care between physician visits.
- Maintains accurate, timely, and complete patient documentation, including patient records, charts, outside appointment information, member meeting notes, communications, special circumstances, and care rendered in the Electronic Medical Record (EMR), while following all related security protocols.
- Implements, monitors, and recommends improvements to care management programs and practices within policies and procedures set by the Lead Physician.
- Assesses, triages, prioritizes, and monitors patient care needs through in-person and telephonic interactions, including brief consultations, active care reviews, high-risk patient follow‑up, and discharge coordination to support continuity of care.
- Administers medications and injections in compliance with state law.
- Provides accurate information to patients and families regarding care and treatment plans, membership benefits, resources, referrals, medications, treatments, authorization requirements, costs for non‑scoped services, and other related issues.
- Performs other duties as required or directed by providers and management.
- Graduate of an accredited school of nursing; bachelor’s degree or professional certification requiring formal education beyond a two‑year college preferred.
- Current Illinois RN license required.
- Current Basic Life Support (BLS/CPR) certification required;
American Heart Association certification preferred.
- 3+ years of nursing experience, including experience with internal medicine nursing tasks; 2+ years of phlebotomy experience required.
- EPIC electronic medical record experience preferred.
