The Licensed Practical Nurse (LPN) provides care to designated patients within the Pennsylvania Licensed Practical Nurse Scope of Practice. Provides care coordination across the continuum. Participates in Network quality improvement initiatives. Demonstrates annual competence with clinical skills, clinical equipment, and electronic medical record system. Performs administrative functions as necessary.
Education- Graduate of an accredited Licensed Practical Nursing Program
- Bachelor’s degree in a health science, or a related field, preferred.
- 1 – 2 years of experience working in an office based medical clinic, preferred.
- Basic Life Support (BLS)
- Strong communication skills. Good time management and organization skills.
- Excellent problem-solving skills.
- Ability to lift and move patients.
- Excellent teamwork and collaboration skills.
- Extensive knowledge of basic health care procedures.
- Knowledge in measuring and administering medication.
- Ability to walk or stand for long periods of time.
- Demonstrates annual competency with clinical skills, clinical equipment, and electronic medical record system.
- Demonstrates knowledge of the problem-solving process in the identification and resolution of issues. Refers problems when they are beyond his or her scope of authority or ability, utilizing the accepted chain of command.
- Demonstrates motivation for learning through independent reading, professional networking, and/or attending relevant conferences & workshops.
- Gives and receives constructive feedback, also listens effectively and is open to opposing points of view.
- Utilizes appropriate and acceptable intervention behaviors and communication styles in order to effectively
- facilitate teamwork.
- Adheres to the VHPCHC Confidentiality policy without exception and consistently takes measures to protect patients Protected Health Information (PHI) per HIPAA guidelines.
- Demonstrates knowledge of & applies patient centered care concepts and respects the rights of patients & their families.
- Identify patients with actual or potential health care needs that would require care. Follows established workflows to close care gaps.
- Actively participates in the Organization’s quality improvement initiatives.
- As requested, willingly accepts and completes in a timely manner, assignments within abilities and professional expertise, to support department and the Organization’s goals and initiatives.
- Maintains a climate of open communication by listening, providing regular feedback, and involving other colleagues in policy, procedure, and/or guideline development. Fosters cooperative spirit, effort, and teamwork.

- Facilitates coordination of care across settings for a designated population, assuring that labs, studies, and referrals are completed and results discussed with the patient. Emphasis on seamless transitions in and out of inpatient settings, including establishing direct linkages to inpatient care teams.
