We are looking for a compassionate
** Licensed Practical Nurse
** to join our growing southern Maine team. This full-time position serves patients throughout
** York, Wells, and nearby communities**.If you are ready to step away from rushed, facility-based care and build meaningful relationships with your patients, home health may be the change you are looking for.##
** What You’ll Do
*** Provide skilled nursing care in patients’ homes under the direction of an RN and physician
* Follow individualized plans of care
* Monitor patients and report changes in condition
* Administer medications and treatments within your scope of practice
* Educate patients and caregivers
* Complete timely and accurate clinical documentation
* Promote patient safety and independence
* Collaborate with nurses, therapists, physicians, and other members of the care team
* Participate in team meetings and the on-call rotation## ##

** What You’ll Need
*** Current and unrestricted Licensed Practical Nurse license in Maine
* Graduate of an accredited practical nursing program
* At least one year of experience in an acute care or comparable clinical setting
* Current CPR certification through the American Heart Association or American Red Cross
* Strong communication, organization, and documentation skills
* Compassionate and patient-focused approach to care
* Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
* Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
