Registered Nurse – Per Diem Assessments and On-Call Visits:

Affirmed Home Care is seeking qualified and dependable Registered Nurses to perform per diem assessments and on‑call nursing visits in patients’ homes. Serving all counties in South Jersey.

This opportunity is ideal for Registered Nurses interested in earning additional compensation while continuing in their current field‑based or full‑time position. Visits may be accepted according to the nurse’s availability.

Job Responsibilities:

Complete start‑of‑care assessments
Conduct supervisory nursing visits
Perform patient reassessments
Complete post‑hospitalization visits
Provide on‑call nursing visits as assigned
Assess patients’ clinical conditions and care needs
Report changes in patients’ conditions to the clinical team
Complete all required visit documentation accurately and on time
Maintain patient confidentiality and follow HIPAA requirements
Comply with Affirmed Home Care policies and applicable nursing regulations

Qualifications:

Active and unrestricted Registered Nurse license
Previous nursing experience required
Home care or field nursing experience preferred
Strong assessment and documentation skills
Ability to work independently in patients’ homes
Excellent communication and time‑management skills
Reliable transportation
Current CPR or BLS certification
