** Locations:
** Home health visits in Ocala area (Marion, Lake, and Sumter counties)

* Schedule:

** Full time; M-F 40 hours with required weekend/ holiday on-call rotations
** What You'll Do
*** Perform comprehensive patient assessments, including OASIS assessments at required intervals
* Develop, initiate, and revise individualized plans of care as patient needs evolve
* Deliver skilled nursing services and treatments as outlined in the plan of care
* Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
* Collaborate with the interdisciplinary team to ensure seamless, holistic care

** What You'll Need
*** Graduate of an approved nursing program with an active Florida RN license
* Prefers BSN or MSN degree
* Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred
* Strong assessment, critical thinking, and patient education skills
* Willingness to participate in weekend and holiday on-call rotations
