After-Hours On Call Registered Nurse (RN)

As an After-Hours On Call RN
, you'll serve as a vital resource for patients, families, and caregivers by providing clinical guidance and support during evenings, weekends, and holidays.
- Triage after-hours patient and family calls, assessing needs and determining appropriate interventions
- Coordinate nursing visits and delegate care to appropriate team members when needed
- Assess physical, emotional, social, and spiritual needs of patients and families
- Collaborate with physicians, interdisciplinary team members, and community resources to ensure seamless care
- Provide symptom management recommendations and update care plans accordingly
- Perform clinical pronouncements of death and offer compassionate support to family members
- Maintain accurate and timely clinical documentation, care plans, and progress notes
- Ensure patients receive the highest level of comfort-focused care throughout their hospice journey
Required:
- Graduate of an accredited Registered Nursing program
- Current Registered Nurse (RN) license in the state of Ohio
- Minimum of two years of professional nursing experience
- Strong clinical assessment, critical thinking, and nursing judgment skills
- Ability to support families during emotionally challenging situations with empathy and professionalism
- Current CPR certification
- Valid driver's license and proof of automobile insurance
