** What you'll need:**
* ** Active LPN or RN License issued by NYS**
* ** Minimum of 6 months experience in a home care setting.**
* ** Current CPR/BLS Certification**
* ** Empathy, Reliability , and a commitment to high-quality care**### #
** What You'll do :*** ##
* ** Provide 1:1 nursing care and treatments according to the patient’s individualized care plan.**
* ** Provide G-tube and PEG tube care, including feedings, medication administration, and site care.**
* ** Perform tracheostomy care and suctioning, and manage ventilator support as ordered.**
* ** Monitor the patient’s condition and promptly report changes to the appropriate healthcare professionals.**
* ** Provide health promotion, counseling, and education to patients and caregivers.**
* ** Administer medications and other prescribed interventions.**
* ** Collaborate with healthcare professionals to coordinate patient care.**
* ** Document patient assessments, care provided, and responses to treatment daily.**
* ** Must be able to lift 50 lbs.**
* ** Must be able to stand and work for extended periods.**
* ** Perform additional responsibilities as required.
