The CDC Foundation is seeking a Nurse Advisor to support the State of Connecticut Department of Social Services (DSS). This position will report to the Deputy Director of Rural Health Transformation for Projects in the Division of Rural Health Transformation in the DSS Commissioner's Office.

The Nurse Advisor will provide clinical and technical expertise to support Connecticut Rural Health Transformation Program (RHTP) project objectives to strengthen health care delivery and coordination focusing on RHTP DSS projects with clinical components.

This position is currently a hybrid position. This role may require statewide travel.

• Active Connecticut Registered Nurse license
• Bachelor of Science in Nursing (BSN)
• Minimum five years of nursing experience.
• Strong written and oral communication skills

• Experience with public health, population health, and related fields
• Experience working in a variety of settings, including one or more of public health, inpatient, outpatient, primary care, behavioral health, geriatrics, and others
• Familiarity with health data, electronic health records systems, and data analysis
• Experience with rural health care

• Experience with rural health care

• Provide clinical input in implementing RHTP projects, including development and adjustment of project design, implementation, monitoring, and evaluation in collaboration with program staff and contractors responsible for implementing RHTP projects.
• Participate in the design, continuous quality improvement, and refinement of RHTP projects.
• Partner with interprofessional leadership at DSS, contractors implementing RHTP projects, and relevant healthcare providers to support improved health outcomes.
• Identify and elevate clinical, operational, and system-level challenges to project implementation to leadership for timely resolution and strategic follow-up.
• In collaboration with contractors implementing RHTP projects, analyze data to monitor performance, evaluate care quality, and inform data-driven decision-making.
• Engage with relevant community partners, healthcare systems, and external stakeholders to support successful program implementation.
